Supported Housing
Cymdeithas Tai Clwyd through the Supported Housing Team provide housing management support to families and individuals who require support to maintain their tenancy successfully. The team works closely with other agencies to provide services.
The Team supports individuals through 4 projects:
Housing Support for Individuals with Mental Illness
- Do you suffer from long term mental illness?
- Do you receive continuous regular support from Tim Dyffryn Clwyd?
- Do you find it difficult to live independently?
- Do you need a safe / suitable place to live in?
- Do you require additional support regarding housing matters?
If you receive support from Tîm Dyffryn Clwyd, ask them to refer you to this scheme.
Support for Homeless People between 16-25 years old
Is this scheme for you?
Yes if you are:
- single
- homeless
- between 16 – 25 years old
How can I be referred to the Scheme?
- Self referral or you can be referred by:
- Denbighshire Housing Department
- Denbighshire Social Services Department.
- Denbighshire Youth Project
Gypsies and Travellers Support Scheme
The team works with Gypsies and Travellers who decide to reside in the housing community, Similar support is available to Gypsies and Travellers who have a Residential Agreement in other areas also.
External Warden Service
Do you receive an element of housing benefit?
If so, it is possible that an External Warden could help you with various aspects of your tenancy for approximately half an hour every day (Monday to Friday). The Warden could assist you with the following:
- Day to day shopping
- Collect prescription
- Visit the bank / Post Office
- Deal with bills / correspondence
- Change bulbs / fuse etc.
- Identify further support needs.
